During stressing on the wall bed, a group of employees were coming in for break and were not paying attention that the bed was being stressed even thou the stressing alarms and lights were on. They were headed to walk by the bed when the flagger on the dead end of the stressing bed stopped them from crossing into the stressing area and stopped the stressing procedure.

Employee was grinding DT edges with a 4" grinder that was not hooked to a vacuum. The grinding was taking place in the building and potions of the building was filling with concrete dust. Spoke with employee and they were not aware if we had a working vacuum. Checking into that. Employee was not wearing a respirator.

Feedback from a few Cemstone drivers is that we need to be more cautious as we walk around the trucks when they are unloading into forms, moving forwards and backwards in the alleyways.
