Tom Peterson | Field (Wells) | Brooklyn Park Data Bank / Job #7275 | 02/02/2021 | 8 | Unsafe Operation | Wood crates continue to be used to transport steel to jobsites. These crates are overloaded causing the bottoms to fail and plates to fall out of them. My fear is that we have a plate go through someone's windshield as they are being transported to a jobsite. I've been told that the metal foundry crates are not getting back to the plants in a timely manner. We need to do a better job of tracking these crates and getting them back sooner so the shops are not forced to use wood. | Needs Attention | View / Edit Details

joe larson | Field (Wells) | fairview community center | 08/06/2020 | 9 | PPE | I Joe Larson saw a driver from Morrel trucking (hauling a 60' JLG boom lift for wells) had safety harness on upside down. as in the leg straps were on his shoulders and the shoulder straps were around legs resulting in "D" ring was between butt cheeks. I saw before he started moving the boom lift and went and talked with him about proper way to wear the harness. he said to me "kinda felt a little weird! he then put the harness on the proper way as I walked away. I turned around as he started machine and saw him attach his lanyard to the top rail of the basket behind him, not a tie off point! I went back and explained where the tie off points were and why not to tie off to rail. I think that any company hauling our lifts should have proper training with the machines they haul for us. he listened and appreciated the help and awareness that I told him. he said his employer gave him harness and never showed him how to use it. also said that lots of times he doesn't wear harness unless someone on job says to. | Needs Attention | View / Edit Details
